Referral Information for Adolescent Substance Abuse/Dual Diagnosis

Referrals are accepted from juvenile courts and the West Virginia Department of Health and Human Resources (WVDHHR). Evaluation materials will be requested if the adolescent appears to meet admission criteria. Once the treatment team determines the appropriateness for admission, an interview will be scheduled

A referral may be made one of three ways:

Referral information should include:

Interview
Upon receipt of the referral a date and time will be arranged to meet with the client for a face-to-face interview. The client may come to Chestnut Ridge Center or our staff is willing to travel to his current placement. Following the interview, a summary with recommendations will be faxed to the referral source.
