Benefits are provided through a group policy issued to WVUH by the Standard Life Insurance Company. You may choose to see any dentist of your choice.

| Orthodontia (children only) | 50% *UCR |
*UCR - Usual Customary Rate. (If the UCR is $100 for a service and the fee was $150, the plan will only pay 80% of the $100.)
If you do not sign up for the dental insurance when you are first eligible, you will be considered a late entrant and will have coverage for preventive services only for the first 12 months. After the 12 months you will have coverage for all services.
**If you choose the dental insurance coverage, you cannot pick up the $250, $600, or $800 dental/vision or child care accounts.
