Benefit-eligible WVU Medicine employees who want to continue using their ID badge for purchases in the cafeterias, Starbucks, Friends Gift Shop, and Medical Center Pharmacy are required to complete a new Assignment of Wages form and turn it in to the Nutrition Services Department. Employees have until Friday, Dec. 15, to submit the form.

Forms must be notarized; therefore, they cannot be mailed to Nutrition Services. A notary will be available when forms are dropped off to the department, which is located in room 4C05 on the fourth floor of J.W. Ruby Memorial Hospital. Office hours are 7 a.m. to 4 p.m.

For weekend or off-shift employees who cannot make it to Nutrition Services during the scheduled times, call 304-598-4105 to make arrangements to have your assignment of wages re-enrolled.

Employees who do not renew by Dec. 15 will have their accounts deactivated effective Dec. 16, 2017. No exceptions.
